Maine Telemedicine Consent Form Information
Permission for Telemedicine Visits with Relief Clinic LLC
What is Telemedicine?
Telemedicine is the process that allows a patient to talk to a provider from the comfort of their home or other chosen location. This is ideal for people that are unable to attend hospitals or clinics due to health reasons or anxiety.
How do I use Telemedicine?
The Telemedicine process runs best on your iOS or Android mobile device. Computers and tablets can be used as well.
How does Telemedicine help me?
The telemedicine process allows you to not have to go into a clinic or hospital to see your provider which decreases your chances of getting sick by others.
Will my Telemedicine visit be private?
- We will not record visits with your provider, however, if people are close to you, they may hear something. You should be in a private place, so other people cannot hear you. We use Telemedicine technology that is designed to protect your privacy.
- If you use the Internet for Telemedicine, use a network that is private and secure, as there is a very small chance that someone could use technology to hear or see your visit.
What if I want an office visit and not a Telemedicine visit?
You are more than welcome to have an office visit if you would prefer it over telemedicine. If that is the case contact your local Relief Clinic provider, or visit our locations pages.
What if I try Telemedicine and I don’t like it?
You can stop using telemedicine any time (even during a Telemedicine visit). From there, you are still able to get an office visit if you no longer want a Telemedicine visit.
Do I have to sign this document?
No, you do not have to sign this document. You will agree to it on the certification page.
What does it mean if I agree to what is in this document?
By checking the box on the certification page that says you agree to the consent form, you are confirming that you have fully read, understood, and agreed to all of the points herein, and still want to continue with the Telemedicine process.
